We've recently updated our online consultation system to Accurx. To submit a request, please use the button below.
Please note: If you normally access our online form through the NHS App, this may take a couple of weeks to update. In the meantime, please use the button above rather than the NHS App to make sure your request reaches us.
If you have any difficulty, please call reception and we will be happy to help.
The NHS App
We would like to invite you to use The NHS App. This allows you to access a range of NHS services. Here is a video which explains how the NHS app works. At this surgery you can use the NHS app to:
- order repeat medication
- nominate a pharmacy
- view medical records

Contact Us Online
We have a new way in which you can contact us online for any routine enquries. We will aim to get back to you within 1 working day. You can use this system to:
- Request sick notes
- Request letters
- Submit a query about prescriptions
- Ask for advice about non urgent medical conditions
If you contact us through ACCURX, you will receive a reply within one working day of submitting your request. (If ordering medication, it may still take 72 working hours).

Proxy Access
As a parent, family member or carer, you may be able to access services for someone else using linked profiles. This is also sometimes called having proxy access. It needs to be set up by the GP surgery where you and the other person are both registered.
